Privacy policy
What this website collects, what we do with it, and the one thing this website is not the right place to send.

Effective August 2026
The short version.
Plain English first. The numbered text below is the full version, and it is the one that governs — but this is the part most people need.
01
What this policy covers
This policy explains how Oxnard Dental Office — “we”, “our”, “the practice” — handles the information you give us through this website. It covers this website only.
It does not cover the dental records we keep for patients we treat. Those are governed by federal HIPAA rules, California medical privacy law, and the practice’s Notice of Privacy Practices, which you receive at the office. If your question is about your dental record, your treatment, or your billing, call the office and ask — that conversation does not belong on a website form.
02
This website is not a patient portal
The appointment request form on this site sends an ordinary internet message. It is not encrypted end to end, it is not a HIPAA-governed channel, and it is not a secure patient portal. We say this plainly because the safe thing to do depends on your knowing it.
Please do not send us any of the following through this website:
- Medical or dental history, symptoms, diagnoses, or treatment details
- Medications, allergies, or conditions
- Photographs, X-rays, or scans
- Insurance member, subscriber, or group ID numbers
- Social Security numbers, dates of birth, or financial account details
- Anything else you would consider confidential health information
Tell us your name, how to reach you, and roughly what you need. We will call you, and anything clinical can be discussed by phone or in the office where it belongs.
Information you submit through this site is not added to your dental chart and is not a substitute for the HIPAA-governed patient records the practice maintains. If you are already a patient, a message sent here does not update your record and does not reach your dentist as a clinical communication.
03
Information we collect
When you fill in the appointment request form, we receive exactly what you enter:
- Your full name
- Your email address
- Your phone number
- Whether you are a new or an existing patient
- Your reason for visiting, chosen from a short list — checkup or cleaning, cosmetic consultation, a filling, crown or repair, an implant consultation, an urgent concern, or other
- Your preferred date, and whether morning, afternoon, or flexible timing suits you
- Anything you choose to write in the optional message box
That is the whole list. This website has no accounts, logins, or passwords, and it never asks for payment details, insurance numbers, or health information.
Like every website, this one is delivered by a hosting provider that keeps standard server logs — IP address, browser and device type, the page requested, and the date and time. We use those only to keep the site available, fast, and free of abuse.
04
How we use it
We use what you send for one purpose: to answer you.
- To contact you about the appointment you asked for, using the phone number or email you gave us
- To answer a question you asked
- To keep an internal record of requests, so nothing is lost between the form and the schedule
- To keep the website working and to block spam submissions
Sending an appointment request does not add you to a marketing list. If the practice ever offers email announcements, joining will be a deliberate choice you make, and every message will carry a one-click way out.
05
Who we share it with
We do not sell your personal information. We do not share it for cross-context behavioral advertising. We do not give it to marketing or promotional partners, and we do not trade or rent contact lists.
A small number of service providers handle information on our behalf and only on our instructions — the company that hosts this website, the service that delivers form submissions to the office, and Google Analytics for visit counts. They may use it to provide that service and for nothing else.
We may disclose information where the law requires it — a subpoena, a court order, or a lawful request from a government authority — or where it is necessary to protect the rights and safety of our patients, our staff, or the public.
07
How we protect information
This site is served over an encrypted connection (HTTPS), so what you type is encrypted in transit. Appointment requests are seen by the staff who schedule visits and are not circulated further.
No transmission over the internet is ever completely secure, and no one can honestly promise otherwise. That is precisely why the rule above exists: keep health details off the website and give them to us by phone or in person, where they are protected properly.
08
Your California privacy rights
California residents have rights under the California Consumer Privacy Act, as amended by the California Privacy Rights Act (CCPA/CPRA). Subject to the limits in the law, you may:
- Know what personal information we have collected about you, where it came from, and what we do with it
- Get a copy of that information
- Correct information that is wrong or incomplete
- Delete the information we hold about you
- Opt out of the sale or sharing of your personal information — we do neither, so there is nothing here to opt out of
- Limit the use of sensitive personal information — we do not collect sensitive personal information through this website
We will never treat you differently, or give you worse service, because you exercised one of these rights.
How to make a request
Call the office at (805) 486-6305, email marioamididmd@gmail.com, or write to us at 3700 Saviers Rd, Oxnard, CA 93033. We may need to verify your identity first, so that we never hand your information to someone claiming to be you. We aim to respond within the 45 days the law allows, and there is no charge.
One distinction worth knowing: information in your dental record is handled under HIPAA and California medical privacy law rather than under the CCPA. Ask the office for the practice’s Notice of Privacy Practices to see how records requests work.
09
Children’s information
We treat children, but this website is written for adults, and a parent or guardian should send any appointment request on a child’s behalf. We do not knowingly collect personal information through this site from anyone under 13. If you believe a child has sent us something here, call us and we will delete it. We do not sell or share the personal information of anyone under 16 — we do not sell or share anyone’s.
10
Links to other websites
Some pages link out — to a map, a review platform, a financing company, or a public health resource. Once you follow one of those links you are on someone else’s website, under their privacy policy and not ours. We do not control those sites and are not responsible for how they handle your information. It is always worth reading the policy of any site you hand information to.
11
Changes to this policy
We may update this policy as the practice and the website change. When we do, we will change the effective date at the top of this page, and we will describe anything material in plain language rather than burying it. This version is effective as of August 2026.

How to reach us
